The Vancouver Coastal Health, Fraser Health and eHealth Strategy Office at UBC want to learn more about your health, lifestyle choices, community involvement and neighbourhood characteristics in order to help improve future community programs and services that are more relevant to your health and wellness needs.

To learn more, they created the My Health My Community survey to give residents the opportunity to help influence their community’s health priorities.

Why the survey? Simple – to improve the health and well-being of the communities they serve. In order to create better local-level programs and policies, they need to better understand the community’s health status and needs.

Who should take the survey? Anyone over the age of 18 that lives in the Metro Vancouver area, Sea to Sky and Sunshine Coast areas.
